Ontario's AI medical transcriber decided to get creative with doctor's notes, hallucinating content that was never actually said. Nothing says 'trust the system' like an AI making up medical information that could end up in patient records. An auditor had to step in to point out what should have been obvious — maybe don't let a hallucination machine handle healthcare documentation.
